The deadline to submit a name change request was Friday January 12th! We are no longer accepting any new requests, as the final list has been turned over to the production team.

As an an investor in either the Reg D or Reg CF offerings for MY DEAD FRIEND ZOE, inclusion in the credits of the film is automatic. So there is nothing extra that you need to do to claim this perk. However, if your name is listed incorrectly or you need to make any changes to the way your name will appear, please use the form below to submit your request.
